Secure patient details

    Send your secure transfer details

Following your enquiry, share your treatment details, blood results, prescriptions and clinical letters here. A clinician reviews them before a consultation booking link is provided. Submitting does not transfer prescribing responsibility or guarantee treatment.

Transfer consultations are free, so each request is checked by a clinician before booking opens. Complete this form and we'll come back to you within two working days with your personal booking link.

    About you

    Your current treatment

    Proof of current treatment

All three of the following are required: your previous blood test results, your previous prescriptions, and your previous clinical letters. This is what our clinician reviews to approve your free consultation — up to 3 files for each, 4MB each.

Files are stored in encrypted, private clinical storage — only your clinical team can open them.

Contacting your GP and previous clinic

Before prescribing, our clinicians need to confirm your treatment history (your diagnosis, previous prescriptions and blood results) with the doctors who have treated you. With your permission, we'll contact them directly. We'll only ask for information relevant to your testosterone treatment, and your consent will be recorded in your notes.

We recommend sharing your GP details — it helps us keep your care joined up and safe. It's optional, so you can still continue without them.

The previous-clinic box must be ticked for us to review your transfer, because your clinician can't prescribe safely without confirming your treatment history. The GP box is optional — if you'd rather we didn't contact your GP, just leave it unticked. You can withdraw your consent at any time by emailing contact@harringtonclinic.co.uk.
